TRT Benefits and Risks: What the Research Actually Shows

Testosterone replacement therapy has gained significant attention in recent years, and with that comes both hype and fear. Here's what the clinical evidence actually shows about the benefits and risks.

Proven benefits

Research consistently shows TRT can improve energy levels and reduce fatigue, increase libido and sexual function, improve mood and reduce symptoms of depression, increase lean muscle mass, reduce body fat percentage (especially abdominal fat), and improve bone mineral density. Most men notice energy and mood improvements within 2-4 weeks. Body composition changes typically become apparent at 3-6 months.

What TRT won't do

TRT is not a performance-enhancing shortcut. It restores testosterone to normal physiological levels — it doesn't push you beyond what's natural. It won't make you lose 50 pounds without lifestyle changes, and it's not a replacement for exercise and good nutrition.

Known risks

Like any medication, TRT carries risks. The most well-documented include elevated red blood cell count (polycythemia) which requires monitoring, potential fertility impact as exogenous testosterone can suppress sperm production, acne or skin changes, and sleep apnea exacerbation in men who already have it. Regular blood work monitoring mitigates most of these risks.

The cardiovascular question

Earlier concerns about cardiovascular risk have been largely addressed by recent large-scale studies. The TRAVERSE trial, published in 2023, found no increased risk of major cardiovascular events in men using TRT. However, individual risk factors should always be discussed with your physician.
